Vehicle information is required for AS, SV and UV permits. Please download and complete this sheet to attach to your submission.

If your vehicle needs a front plate, you must submit a Departmental FLP Order Form.

Project ID accuracy/use is the responsibility of the requesting department.

Transportation Account Numbers are the same as in previous years, unless otherwise notified by our office, and begin with the GN or F. This number is not the same account number used with the Guest Online Permit System.

All departmental permits for FY 26-27 are valid through June 30, 2027.
